Vanguard care model set to push older skilled nursing homes to brink

AUSTIN: The Vanguard care model is going to push older skilled nursing homes faster to the brink.

There are three types of model: MCPs (multi-specialty community providers), concerned with moving specialist care out of hospitals and into the community; PACs (primary and acute care system), with single organisations providing hospital, GP and community services; and enhanced health in skilled nursing homes. These models are meant to offer more joined-up care, health and rehabilitation services.

But the skilled nursing homes will be technology driven mini-hospitals and only a handful of the current outlets come close in qualifying to service the emerging trend…
