228: Health inspectors have branded a care provider unsafe after a patient was found on the floor with injuries and was taken to hospital.

Staff had missed both evening and morning visits at the patient’s home before he was found lying on the floor during a scheduled lunch time visit.

The report said that staff were repeatedly late or missed call outs to patients.
